[?] A summary of the violations found during the inspection are listed below.

3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical Repeat Corrected During Inspection TCS foods not being cold held at the proper temperature. Observed cole slow, caramelized onions and lettuce leaves holding at about 51F. PIC discarded the onions and lettuce leaves and stated the cole slow has been there for 2 hours. PIC put the cole slow as well as all TCS food in the make table on an ice bath.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Equipment components are not intact, tight or properly adjusted. Observed the make table in the kitchen not working properly. PIC stated maintenance will come to check the cooler.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Facility not maintained clean. Observed the ouside area of the facility needing cleaning as well as the floor inside the facility.

Complainant stated being at the facility a couples of times within the past couple of weeks and had some observations: Ordered cole slows that were warm, observed the outside of the facility is getting unclean and the inside walls and floor need cleaning, observed employee coming in with apron on and smoking then throw cigarette on floor before coming in, observed waitress showing sign of cold or flu.
We went to the facility and talked to the manager. Observed the outside of the facility needing cleaning and the floor needing cleaning as well. PIC stated a request has been made to have someone come in and duck scrub the entire floor of the kitchen. PIC stated a company come in and power wash the outside of the facility and that a request will be put in to have them come an clean the outside. PIC stated employees are aware of not smoking with their Apron on as well as taking them off while on break or going to the restroom. PIC stated employees are also aware of not working around food while sick. Reviewed illness policy with PIC and reviewed proper designated areas for smoking break as well as proper use of aprons. Observed cole slow cold holding at 51F. PIC took it off the cooler. Cooler needs to be checked, some food items are holding at 41F or below and some food items are not. Violation regarding the complaint are cited on this report.
