Critical Employees are not informed in a verifiable manner of their responsibility to report information about their health. Facility has an employee sick policy but sick policy signed by the employees is missing Norovirus and some symptoms.

3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Repeat Equipment food-contact surfaces or utensils are unclean. Observed the ice shoot at the self service drink fountain and at the window with build up. Also observed build up in the ice machine in the kitchen.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Equipment components are not intact, tight or properly adjusted. Observed torn gaskets on the door of the 4 door Traulsen reach in cooler in the kitchen and on the door of he 2 door reach in cooler at the front.

Equipment components are not intact, tight or properly adjusted. Observed ice build up in the 2 door reach in freezer in the kitchen. Observed ice build up on the door of the walk in freezer and condensation on the ceiling.

Unclean nonfood-contact surfaces. PIC stated drink nozzles are cleaned 2 to 3 times a week. Drink nozzles have to be cleaned once a day.

Non-food contact surfaces of equipment are unclean. Observed debris in the gaskets of several coolers and freezers. Observed build up on the microwaves in the kitchen. Observed build up on the coffee creamer dispenser by the window.

No drain plugs in waste receptacle. Observed no drain plug on the trash dumpster.

Waste receptacles not covered properly. Observed dumpsters lid open.

III - P - Preventing Contamination by Hands: Food employees were not contacting exposed ready-to-eat foods with bare hands.
IV - P - Demonstration of Knowledge: The person in charge is Certified in Food Protection.
VI - P - TCS Food: Observed time/temperature controlled for safety foods being properly held using time as a public health control; the operation had a written procedure on file, foods were properly marked to indicate the time that the food was removed from temperature control, and food was discarded when required.
VII - Protection from Contamination: Equipment food-contact surfaces or utensils are dirty. Observed build up on ice shoots and ice machine.
