Critical Employees are not informed in a verifiable manner of their responsibility to report information about their health. Employee health policy was not verifiable at the time of inspection. Facility will be provided with a copy of the Marion Public Health Sick policy.

Equipment components are not intact, tight or properly adjusted. Observed the 2 door reach in freezer in the kitchen not working properly. PIC moved all food items to the walk in frezzer. Maintenace was at the facility to check the freezer at the time of inspection.

Corrected During Inspection Non-food contact surfaces of equipment are unclean. Observed accumulation of food debris at the bottom of the 2 door freezer in the kitchen as well as inside the gaskets.
