Critical Person in charge did not ensure that employees are informed of their responsibility to report information about their health. A defined or written employee health policy was not in place.
To prevent transmission of foodborne illness, the person in charge shall ensure that food and conditional employees are informed of their responsibility to report information about their health as it relates to diseases that are transmissible through food. Critical Observed no towels or hand drying device at the handwashing sink(s).
To prevent contamination by hands, each handwashing sink or group of adjacent handwashing sinks shall be provided with: Individual, disposable towels; a continuous towel system that supplies the user with a clean towel; a heated-air hand drying device; or a hand drying device that employs an air-knife system that delivers high velocity, pressurized air at ambient temperatures. Observed waste receptacles that were not covered. The dumpster lid was not closed.
To prevent attracting pests, Receptacles and waste handling units for refuse, recyclables, or returnables shall be: kept covered inside the FSO or RFE if they contain food residue and are not in continuous use, and after they are filled; or with tight-fitting lids or doors if kept outside. 3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed a build-up of dirt and debris. The milk shelves are dirty.
To prevent attracting pests or contaminating food containers, The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed. CLEAN THE SHELVES THAT HOLD THE MILK. Correct By: 14-Mar-2015
3717-1-06.4(F) / Drying mops.
Observed mops being dried using an unapproved method. Mop was in bucket after use.
After use, mops shall be placed in a position that allows them to air-dry without soiling walls, equipment, or supplies. HANG UP MOP AFTER USE. Correct By: 08-Mar-2015
