3717-1-03.1(R)(3) / Shellstock - retaining the identity of the source.
Critical Repeat CorrectedDuringInsp The identity of the source of shellstock being served or sold was not retained. Observed raw oysters with no shellstock tags. Employee stated they are for personal use only. Employees shall keep personal food separate from consumer food & easily identifiable to prevent a contamination which could cause a food borne illness. CORRECTED: Employee made space for personal food ,separate from consumer food.
To prevent illness, the identity of the source of shellstock that are sold or served shall be maintained by retaining shellstock tags or labels for ninety calendar days from the date that is recorded on the tag or label by using an approved record keeping system as required by this rule. 3717-1-02.4(A)(2) / Person in charge: assignment of responsibility.
Repeat Facility did not have one employee with supervisory and management responsibility and the authority to direct and control food preparation and service with level two certification in food protection. Has Level 2 just not the Ohio Certification. Gave another reciprocity paper or that they shall retest and get Ohio Certification Owner sent in paper work for reciprosity to Ohio.
At least one employee that has supervisory and management responsibility and the authority to direct and control food preparation and service shall obtain the level two certification in food protection according to rule 3701-21-25 of the Administrative Code. Correct By: 19-Jan-2018
