Observed unapproved materials covering the walls or ceiling. The corners on the wall between the oven and bathroom are not covered making cleaning difficult.
Wall and ceiling covering materials shall be attached, finished and sealed to provide a smooth, nonabsorbent, easily cleanable surface. INSTALL CORNER COVERS ON THE WALL BETWEEN THE OVEN AND BATHROOM. Correct By: 24-Jul-2015

PERSON IN CHARGE (PIC) – The PIC was aware of major food allergens (NUTS, PEANUTS, SOY, MILK, FISH, SHELLFISH, WHEAT, EGGS), requirements for cooking (PIZZA-165) holding temperatures (41), date marking (7 DAYS OR LESS), thawing (UNDER REFRIGERATION), symptoms of illness (DIARRHEA, VOMITING, SORE THROAT WITH FEVER, JAUNDICE), and sanitizer strength (150-400 PPM QUAT).

EMPLOYEE HEALTH – A written policy is available. PIC was knowledgeable of symptoms employees must not work with (diarrhea, vomiting, sore throat with fever, or jaundice). Employees are aware of symptoms and responsibility to report to PIC. PIC knows to report doctor confirmed foodborne illnesses to the health department. A sign off sheet for all employees has been created.
