Critical Repeat Observed food packages that are not properly labeled. The New York Strip Steak and Ground Chuck were not labeled correctly.

A label on a food packaged in the RFE or FSO did not contain all required information. The cheese that was packaged in the store did not contain all the required ingredients.
Foods shall be labeled as specified in this rule. PLACE THE INGREDIENTS ON THE LABEL OF THE CHEESE. EACH INDIVIDUAL TYPE OF CHEESE THAT IS IN THE PACKAGE MUST HAVE THE INGREDIENTS LISTED. Correct By: 13-May-2014
3717-1-04.5(A)(3)/Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are dirty. The coolers in the back room had dirty a shelf (floor of cooler).
Nonfood-contact surfaces of equipment shall be kept clean. CLEAN THE FLOOR OF THE COOLERS THAT ARE IN THE BACK ROOM. Correct By: 19-May-2014

Observed employees lockers located where food, equipment, or utensils may be contaminated. Employee lunches were stored on the third shelf in the walk in cooler.
Lockers or other suitable facilities shall be located in a designated room or area where contamination of food, equipment, linens, and single-service and single-use articles cannot occur. USE THE LOWEST SHELF IN THE WALK IN COOLER TO KEEP EMPLOYEE FOOD SEPARATE FROM FOOD FOR SALE AND TO PREVENT CONTAMINATION OF THE SALES FOOD. Correct By: 12-May-2014
3717-1-06.4(F)/Drying mops.
Observed mops being dried using an unapproved method. The mops were in the mop bucket and on the floor of the mop sink.
After use, mops shall be placed in a position that allows them to air-dry without soiling walls, equipment, or supplies. HANG THE MOPS UP AFTER USE TO LET THEM DRY. Correct By: 13-May-2014
901:3-4-15(B)/Embargoing of food due to adulteration, misbranding, or expired baby formula or baby foods
Food in an RFE must be embargoed due to adulteration, misbranding, or baby food or formula is expired. Baby Food was outdated. Six 1 quart Silimac Soy were dated May 1, 2014.
Whenever a licensor finds or has cause to believe within an RFE or FSO in their jurisdiction that any food is adulterated, or so misbranded as to be dangerous or fraudulent, or that infant formula, or baby food is expired, the licensor shall affix to the item a tag giving notice that the item is, or is suspected of being, adulterated, misbranded, or expired and has been embargoed. DO NOT OFFER FOR SALE OUTDATED BABY FOOD. THE BABY FOOD WAS DESTROYED AT THE TIME OF INSPECTION BY DUMPING IT DOWN THE MOP SINK. Correct By: 12-May-2014

Corrections have been made from the May 12, 2014 inspection
-Labels on the packaged mixed cheeses contains the proper labeling - contains the ingredients of each type of cheese.
-The floor of the cooler is clean.
-The employees lunches are stored on the lowest two shelves in the walk in cooler. They are not above foods offered for sale.
-The mops are hung up to dry.
-No expired baby food was seen

The raw New York strip Steak and Ground Chuck are still not labeled corrected. However, ODA has been notified of the problem and they are working on correcting the problem.
