3717-1-03.2(A)(2) / Preventing contamination from hands - bare hand contact
Critical Repeat Observed food employee touching ready-to-eat food with bare hands. The ice scoop was not used to put ice in the cup. The cup was scooped into the ice.
Except when washing raw fruits and vegetables, food employees may not contact exposed, ready-to-eat food with their bare hands and shall use suitable utensils. To prevent possible cross contamination from hands to ice, USE THE ICE SCOOP TO PUT ICE IN THE CUPS. THIS WAS ADDRESSED AT THE TIME OF INSPECTION. Correct By: 15-Feb-2014

3717-1-03.2(M) / Wiping cloths - use limitation.
Repeat Observed improper use and/or maintenance of wiping cloths. Several wiping cloths were left on the counter.
cloths in use for wiping counters and other equipment surfaces shall be held between uses in a chemical sanitizer, or laundered daily; KEEP THE WIPING CLOTHS IN THE SANITIZER BUCKETS BETWEEN USES. DO NOT LEAVE THEM ON THE COUNTERS. Correct By: 15-Feb-2014

3717-1-04.6(C)/Hot water and chemical sanitizing - methods.
Food-contact surfaces and utensils were not sanitized using an approved combination of exposure time, chemical concentration, temperature, and/or pH.
After being cleaned, food-contact surfaces and utensils shall be sanitized by a method as listed in this rule.
3717-1-06.4(B)/Cleaning - frequency and restrictions.
Observed a build-up of dirt and debris.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.
3717-1-06.1(M)/Outer openings - protected.
Observed an outer opening, window, or door without proper protection against the entry of insects and rodents.
To prevent contamination, outer openings of an FSO or RFE shall be protected against the entry of insects and rodents as specified in this rule.
3717-1-04.4(S)(2)/Single-service articles and single-use articles - bulk milk container dispensing tube
The dispensing tube on bulk milk container was not properly cut as required. The tube from the creamers were cut straight across.
The bulk milk container dispensing tube shall be cut on the diagonal leaving no more than one inch protruding from the chilled dispensing head. CUT TUBING FROM THE CREAMERS ON A DIAGONAL AND NO MORE THAN 1 INCH LONG. Correct By: 15-Feb-2014
Comments
The following have been corrected since the last inspection.:
-The gap between the bottom of the back door and the floor is sealed. The door fits tightly in the frame and there are no gaps.
-The walk in freezer is clean.
-The dishwasher is dispensing sanitizer.
