Critical Insufficient air gap between the flood rim and the water supply inlet. OBSERVED: Under the coffee maker and in the walk in cooler- needs an air gap to prevent the back siphoning of contaminated water into potable water which could cause a foodborne illness.

Floors, walls, and/or ceilings not smooth and easily cleanable. OBSERVED: Missing/broken floor tiles, ie: front check out, back by restroom etc. . .Replace broken tiles as needed throughout facility.

Facility not maintained clean. OBSERVED: Ceiling vents with dirt / debris. To prevent contamination from the premises, clean ceiling vents.
