Critical PIC unable to demonstrate knowledge by having no critical violations. OBSERVED: Critical violations in facility. Facility shall have a manager with knowledge of the food code to prevent critical violations which could cause a foodborne illness. Provide manager with education.

Critical CorrectedDuringInsp TCS foods not being cold held at the proper temperature. Observed: Cold meats in Norlake & serving line cold holding unit at 54 degrees. To prevent a foodborne illness. TCS foods shall be kept at 41 degrees or below to prevent the growth of pathogens. Discard TCS foods out of temperature in both units. CORRECTED: Employee removed & discarded unsafe TCS foods

CorrectedDuringInsp Hot and cold holding equipment thermometer was missing, located incorrectly, or not easily readable. OBSERVED: Norlake thermometer in the rear of the unit. So employees may read & notice when units may be out of temperature. Thermometers shall be placed at the warmest spots in the front of the unit. CORRECTED: Employee placed thermometer in front of unit

3717-1-04.3(B) / Fixed equipment installation - spacing or sealing.
Fixed equipment not properly sealed or spaced for cleaning. OBSERVED: Walk in cooler with water inside on the floor. Appears the seal is incorrect & cause condensation on the floor. Unit does not shut and seal properly. Replace / repair seal to fix the problem.

Nonfood-contact surfaces not easily cleanable.OBSERVED: Ceiling fans are dirty. To prevent a contamination to food. food contact surfaces. All surfaces shall be kept free of dirt.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean. OBSERVED: Bread rack, oven & other equipment with food debris on the equipment. To prevent a contamination from dirty surfaces, all surfaces shall be kept clean, smooth & easily cleanable. Clean as needed.

Cooking equipment or pan surfaces unclean. OBSERVED: Cutting board that was stained & had cut marks in it. To prevent a foodborne illness. Plane or replace cutting board. All surfaces shall be smooth & easily cleanable.

Unnecessary or nonfunctional items and /or litter on premises. OBSERVED: Drink machine, old grill parts? No longer being used. To prevent clutter remove & store unused equipment.

Repeat Facility not maintained clean. OBSERVED: Floors throughout facility not maintained in a clean manner. To prevent contamination, floors and facility shall be ,maintained in a clean manner. Clean as needed.

3717-1-06.1(C) / Floor and wall junctures - coved, and enclosed or sealed.
Floor and wall junctures not properly coved or closed and/or floor drains not provided. OBSERVED: Coving off around the freezer & different places on wall junctures. Replace missing coving. All surfaces shall be smooth & easily cleananble.

3717-1-06.4(F) / Drying mops.
Mops dried improperly. OBSERVED: Mop not hanging to dry. Wash in mop sink. To prevent the spread of pathogens mops shall be hung to air dry. Remove from the sink & hang up.
