Repeat Fixed equipment did not appear to be properly sealed or spaced for cleaning. The three compartment sink is not attached to the wall.
Equipment that is fixed shall be installed so that it is spaced to allow access for cleaning, from adjoining equipment, walls, and ceilings a distance of not more than one thirty-second inch or one millimeter; or sealed to adjoining equipment or walls, if the equipment is exposed to spillage or seepage; Counter-mounted equipment shall be installed to allow cleaning of the equipment and areas underneath and around the equipment as specified in this rule. Observed food that was not properly protected from contamination by separation, packaging, and segregation. The roast was on top of the box of celery, the steak was store over the dressing, and other raw meat was stored over the ketchup in the walk in cooler.
To prevent contamination with pathogens that could cause a foodborne illness, food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from cooked ready-to-eat food as described in this rule. STORE ALL RARE MEAT ON SHELVES LOWER THAN READY TO EAT FOOD. THE STEAK, ROAST AND OTHER MEAT WAS RE-ARRANGED ON THE SHELVES AT THE TIME OF INSPECTION. Correct By: 06-Dec-2014

Observed improper storage of food items. A box of celery and box of roast beef were stored on the floor.
Food shall be protected from contamination by storing the food in a clean, dry location, where it is not exposed to splash, dust, or other contamination, and at least six inches above the floor. STORE ALL FOOD ON SHELVES AND/OR AT LEAST 6 INCHES OFF OF THE FLOOR. Correct By: 06-Dec-2014
3717-1-06.1(I)/Light bulbs - protective shielding.
Observed light bulbs or heat lamp that were not properly shielded or coated where required. The light over the bar sink was not shielded.
Light bulbs shall be shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, or linens, or unwrapped single-service or single-use articles and does not apply in areas used only for storing food in unopened packages. An infrared or other heat lamp shall be protected against breakage by a shield surrounding and extending beyond the bulb so that only the face of the bulb is exposed.
