CorrectedDuringInsp Facility does not have an employee with level two certification in food protection. OBSERVED; Person with Level 2 does not have Ohio certification. Manager found the Ohio Version for level 2

3717-1-03.4(A) / Frozen food - temperature and time control.
CorrectedDuringInsp Foods in frozen storage exhibit signs of thawing or refreezing. OBSERVED: Fish, poultry & beef that were in different forms of thawing. To prevent a foodborne illness frozen food shall remain frozen. CORRECTED: Manager removed & discarded partially thawed frozen food.

3717-1-04.4(Q)(2) / Utensils, temperature and pressure measuring devices - good repair and calibration.
Utensils, thermometers, and/or pressure gauges not in good repair or calibrated. OBSERVED : SMALL center aisle cooler with cheese with non working thermometer. Provide a working thermometer in the unit.

3717-1-04.5(D) / Nonfood-contact surfaces - cleaning frequency.
Non-food contact surface(s) not cleaned at the required frequency. OBSERVED: Food debris & dirt on long refrigeration units on north wall. Clean the units & shelving.

3717-1-06.1(A) / Floors, walls, and ceilings.
Floors, walls, and/or ceilings not smooth and easily cleanable. OBSERVED: Missing or broken floor tiles behind the meat self service case & under the frozen self service case. Replace as needed so all surfaces are smooth & easily cleanable.
