3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Equipment food-contact surfaces or utensils are dirty. The inside of the ice machine is slightly moldy.
To prevent contamination of ice with pathogens that could cause a foodborne illness, equipment food-contact surfaces and utensils shall be clean to sight and touch. CLEAN AND SANITIZE THE INSIDE OF THE ICE MACHINE. CLEAN BEFORE USING AGAIN. Correct By: 29-Dec-2014

Repeat Equipment is not approved by a recognized testing agency. The microwave ovens, grills, and small frier are for household use only.
Food equipment that is acceptable for use in a FSO or RFE shall be approved by a recognized food equipment testing agency. REPLACE/REMOVE THE RESIDENTIAL MICROWAVE OVENS, GRILLS, AND GRAY FRIERS. Correct By: 10-Jun-2015
3717-1-06.1(A) / Floors, walls, and ceilings.
Floors, walls, and/or ceilings were not smooth and easily cleanable. The basement in the area of the freezers has brick walls and raw wood.
Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. THE WALLS, FLOORS, AND CEILING IN THE BASEMENT IN THE AREA OF THE FREEZERS MUST BE COVERED WITH A SMOOTH EASILY CLEANABLE SURFACE. CORRECT AFTER THE RESIDENTIAL EQUIPMENT HAS BEEN REPLACED.
3717-1-06.1(I) / Light bulbs - protective shielding.
Observed light bulbs or heat lamp that were not properly shielded or coated where required. The light cover is missing on the light over the bar.
Light bulbs shall be shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, or linens, or unwrapped single-service or single-use articles and does not apply in areas used only for storing food in unopened packages. An infrared or other heat lamp shall be protected against breakage by a shield surrounding and extending beyond the bulb so that only the face of the bulb is exposed. EMPLOYEE HEALTH – PIC was knowledgeable of symptoms employees must not work with (diarrhea, vomiting, sore throat with fever, or jaundice). Employees are aware of symptoms and responsibility to report to PIC. PIC knows to report doctor confirmed foodborne illnesses to the health department. A model written policy was provided.

PERSON IN CHARGE (PIC) – The PIC was aware of major food allergens, requirements for cooking, cooling, date marking, thawing, symptoms of illness, sanitizer strength, and storage order.

LEVEL 1 FOOD SAFETY TRAINING – At this facility, PICs on all shifts ARE NOT REQUIRED, but encouraged to be certified in Level 1 Food Safety.

FOOD DEFENSE - The food dense checklist (NYSHD’s Food Defense Strategies) was provided.
