3717-1-03.4(I)(1) / Time as a public health control - written procedures
Critical Time as a public health control was being used without the proper written procedures. Although there was a written procedure for the time making of the tomatoes, there was not one for the lettuce.
If time without temperature control is used for TCS food before cooking, or for ready-to-eat TCS food that is displayed or held for sale or service, written procedures shall maintained in the FSO or RFE and made available to the licensor. To prevent the multiple grow of pathogens that coud cause a foodborne illness, OBTAIN THE WRITTEN PROCEDURE FOR TIME MARKING OF THE LETTUCE. THE MANAGER WAS LOOKING INTO THIS AT THE TIME OF INSPECTION. Correct By: 19-Apr-2014
3717-1-05.1(B)(1) / Approved plumbing system.
Critical CorrectedDuringInsp The plumbing system does not comply with the Ohio building code. The drain from the ice machine is touching the floor.
A plumbing system shall be designed, constructed, and installed according to the Ohio building code. To prevent a cross contamination between the sewer and the ice machine from a sewage back up which could cause a foodborne illness, ADJUST THE DRAIN FROM THE ICE MACHINE SO THAT THERE IS AN AIR GAP BETWEEN THE FLOOR DRAIN AND THE ICE MACHINE DRAIN. THIS WAS TEMPORARILY CORRECTED AT THE TIME OF INSPECTION BY PLACING A WEDGE UNDER THE DRAIN SO THAT THE END OF THE DRAIN WAS OFF OF THE FLOOR.. Correct By: 19-Apr-2014

3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Equipment and/or components are not maintained in good working order. The door to the under counter cooler between the front counter and the grill does not seal properly. The hinge is damaged.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code. ADJUST THE DOOR TO THE COOLER SO IT SEALS THE DOOR TO THE FRAME. Correct By: 25-Apr-2014

Observed an improperly cleaned storage area for refuse, recyclables, or returnables. The dumpster was dirty and grease covered.
A storage area and enclosure for refuse, recyclables, or returnables shall be maintained clean and free of unnecessary items. CLEAN THE DUMPSTER MORE FREQUENTLY. Correct By: 25-Apr-2014

The physical facilities are not being maintained in good repair. There is peeling paint under the hood.
