The physical facilities shall be maintained in good repair. REPAIR OR REPLACE THE MISSING FLOOR TILES, FLOOR PAINT IN THE DISHROOM SO THAT IT IS SMOOTH AND EASILY CLEANABLE AND TO PREVENT BACTERIAL GROWTH. Correct By: 12-Dec-2016

Repeat Observed plumbing fixtures that were not being kept clean and maintained. OBSERVED A BUILD UP OF DIRT AND DEBRIS UNDER THE FOOD GRINDER.
Plumbing fixtures such as handwashing sinks, toilets, and urinals shall be cleaned as often as necessary to keep them clean and maintained. CLEAN THE AREA UNDER THE FOOD GRINDER.
