To prevent foodborne illness, no person, firm, association, organization, corporation, or government operation shall construct, install, provide, equip, or extensively alter a food service operation until the facility layout and equipment specifications have been submitted to and approved in writing by the licensor. 3717-1-04.2(B)(2) / Manual warewashing - sink compartment size requirements.
Critical Sink compartments were too small to accommodate the largest piece of equipment. The three compartment sink is not available in the proposed food prep area. The sinks behind the bar are not large enough.
Sink compartments shall be large enough to accommodate the largest equipment and utensils. If equipment or utensils are too large for the warewashing sink, a warewashing machine or approved alternative shall be used. INSTALL A THREE COMPARTMENT SINK IN THE AREA THAT CAN BE USED BY THE FOOD PREP AREA. Correct By: 13-Oct-2014

3717-1-03.2(K) / In-use utensils - between-use storage.
In-use utensils are improperly stored. The ice scoop handle was laying in the ice.
During pauses in food preparation or dispensing, food preparation and dispensing utensils shall be stored as described in this rule. STORE ICE SCOOP HANDLE OUT OF ICE.
3717-1-04.1(KK) / Food equipment - certification and classification.
Equipment is not approved by a recognized testing agency. The toaster oven is not approved for commercial use. It is for household use only.
Food equipment that is acceptable for use in a FSO or RFE shall be approved by a recognized food equipment testing agency. REMOVE THE TOASTER OVEN FROM THE FACILITY. SUBMIT A LIST OF EQUIPMENT THAT IS IN THE FOOD PREP AREAS. INCLUDE THE MAKE AND MODEL NUMBERS. Correct By: 13-Oct-2014
3717-1-04.1(Y) / Temperature measuring devices.
Cold or hot holding equipment did not contain a thermometer, or the thermometer was not correctly placed. The bar cooler with milk does not have a thermometer.
A properly designed temperature measuring device as specified in this rule shall be located in the warmest part of a cooling unit or the coolest part of a food warming unit. INSTALL A THERMOMETER IN THE BAR COOLER. Correct By: 26-Sep-2014
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Equipment and/or components are not maintained in good working order. The floor of the walk in cooler is rusty and water is leaking onto floor.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code. REPAIR THE LEAK IN THE WALK IN COOLER AND RESURFACE THE FLOOR TO COVER THE RUST. Correct By: 13-Oct-2014
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Equipment and/or components are not maintained in good working order. The insides of both beer coolers are dirty.

Observed no service sink or curbed cleaning facility as required. A mop sink is not available.

3717-1-05.1(S)(2) / Plumbing system - maintained in good repair.
The plumbing system was not properly maintained. The air gap on the ice bin is not properly aligned which is cause the water from the ice bin to drip in a bucket and not into the drain pipe.
A plumbing system shall be properly maintained. RE-ALIGN THE DRAIN FROM THE ICE BIN SO THAT IT DRAINS INTO THE OUTLET PIPE. Correct By: 26-Sep-2014
3717-1-05.1(S)(2) / Plumbing system - maintained in good repair.
The plumbing system was not properly maintained. The toilet in the women's bathroom does not work.
A plumbing system shall be properly maintained. REPAIR THE TOILET IN THE WOMEN'S BATHROOM.
3717-1-05.4(F)(1) / Outside receptacles - tight fitting lids
Observed outdoor refuse containers without tight fitting lids, doors, or covers. The Dumpster lid was not closed.
Receptacles and waste handling units for refuse, recyclables, or returnables used outside with materials containing food residue, shall be designed and constructed to have tight-fitting lids, doors, or covers. KEEP THE DUMPSTER LIDS CLOSED. Correct By: 12-Sep-2014
3717-1-06(A)(1) / Indoor areas - surface characteristics
Observed floors, walls, and/or ceilings that were not constructed of approved materials. The area used for food prep is not smooth, easily cleanable and durable.
Where FSO or RFE activities are conducted, materials for indoor floor, wall, and ceiling surfaces shall be smooth, durable, and easily cleanable; closely woven and easily cleanable carpet for carpeted areas; and nonabsorbent for areas subject to moisture. Observed no handwashing sign(s) posted at handwashing sink(s) used by employees. The bar hand sink and the bathroom hand sinks do not have signs.

3717-1-06.2(I)(1) / Lighting - intensity
The light intensity in the walk-in refrigerator, dry food storage area, or other area during cleaning was less than ten foot candles. The lighting in the walk in cooler is not bright enough.
The light intensity shall be at least ten foot candles (one hundred eight lux) at a distance of thirty inches (seventy-five centimeters) above the floor, in walk-in refrigeration units and dry food storage areas and in other areas and rooms during periods of cleaning. The light intensity in an area where an employee was working with food, utensils or equipment was less than fifty foot candles. The area that was used as the food prep area is not bright enough.
The light intensity shall be at least fifty foot candles (five hundred forty lux) at a surface where a food employee is working with food, utensils or equipment such as knives, slicers, grinders, or saws where employee safety is a factor.
