Critical There is no test kit available for measuring the concentration of the sanitizer.
A test kit or other device that accurately measures the concentration in PPM (mg/L) of sanitizing solutions shall be provided. To ensure that the sanitizer strength is adequate to destroy pathogens, OBTAIN TEST STRIPS FOR THE QUAT SANITIZER AND CHECK SANITIZER STRENGTH REGULARLY. TEST STRIPS WERE LOANED TO THE OPERATOR AT THE TIME OF INSPECTION. Correct By: 14-Jan-2014

Observed improper storage of food items. The chicken wings were stored under the bottom shelf in the walk in cooler.
Food shall be protected from contamination by storing the food in a clean, dry location, where it is not exposed to splash, dust, or other contamination, and at least six inches above the floor. STORE THE CHICKEN WINGS ON THE SHELVES AT LEAST 6 INCHES OFF OF THE FLOOR. Correct By: 14-Jan-2014

The physical facilities are not being maintained in good repair. The bathroom door does not close and latch on its own
