Equipment is not approved by a recognized testing agency. The microwave oven, grills and small frier are for household use only.
Food equipment that is acceptable for use in a FSO or RFE shall be approved by a recognized food equipment testing agency. REMOVE/REPLACE THE MICROWAVE OVEN, GRILLS, AND GRAY FRIER. Correct By: 10-Jun-2015
3717-1-04.1(Y) / Temperature measuring devices.
Cold or hot holding equipment did not contain a thermometer, or the thermometer was not correctly placed. The bar cooler and the 2 door cooler do not have working thermometers.
A properly designed temperature measuring device as specified in this rule shall be located in the warmest part of a cooling unit or the coolest part of a food warming unit. PLACE THE THERMOMETER IN THE BAR COOLER AND IN THE 2 DOOR KITCHEN COOLER. Correct By: 28-Aug-2014
3717-1-06(A)(1) / Indoor areas - surface characteristics
Observed floors, walls, and/or ceilings that were not constructed of approved materials. The basement in the area of the freezers has brink walls and raw wood.
Where FSO or RFE activities are conducted, materials for indoor floor, wall, and ceiling surfaces shall be smooth, durable, and easily cleanable; closely woven and easily cleanable carpet for carpeted areas; and nonabsorbent for areas subject to moisture. THE WALLS, FLOORS AND CEILING IN THE BASEMENT IN THE AREA OF THE FREEZERS MUST BE COVERED WITH A SMOOTH EASILY CLEANABLE SURFACE. CORRECT AFTER THE RESIDENTIAL EQUIPMENT HAS BEEN REPLACED. FOOD DEFENSE - A food defense booklet (NYSHD’s Food Defense Strategies) and possible vulnerable points to protect food from intentional contamination were briefly discussed. (Facility security, employee training and restrictions, and food storage, handling, and control were reviewed).

EMPLOYEE HEALTH - A model health policy was provided to the PIC and reviewed. Employees may not work with the food when ill with diarrhea, vomiting, sore throat with fever, or jaundice. Report to the health department if an employee has a doctor confirmed foodborne illness.

PERSON IN CHARGE (PIC) – Printed information about allergy awareness, critical food handling items, and level 1 food safety training was provided to the person in charge (PIC).

Violations that have been corrected since the 10-23-14 inspection.
-the coolers and food in the kitchen are 41 degrees or less.
-the inside of the ice machine is clean.

The pop box storage location must be protected from contamination.
