Observed no ODH certified manager/supervisor in food protection. Facility does not have an employee with manager certification in food protection. Obtain certification within 3 months.

Observed foam to go containers, plates, and covers not stored inverted. Single-service and single-use articles not protected from contamination. Store/stack items inverted, upside down.

3717-1-04.4(B) / Cutting surfaces.
Observed cutting boards to be scratched/scored beyond being able to be effectively sanitized. Cutting blocks or boards cannot be effectively cleaned and sanitized. Resurface or replace cutting boards.

Observed wood shelves in dry storage room to not be smooth and easily cleanable. Nonfood-contact surfaces not easily cleanable. Resurface or paint wood shelves to make smooth and easily cleanable.

Observed not test kit to test sanitizer in dish machine. Sanitizer test kit or other device not being used. Obtain test kit. Note: manager has a chlorine test kit on order.

Observed food soilage on handles and front of coolers in the kitchen. Non-food contact surfaces of equipment are unclean. Clean handles and fronts of coolers.

Observed spray nozzle in dish machine area leaking. Plumbing system not properly maintained or repaired. Repair leaking sprayer and maintain in good working order.

Observed soiled mop sitting in mop bucket between use. Mops dried improperly. Hang mop to dry between use.

Complaint received 7/5/23: Shelby Pointe is infested with flies and maggots in the kitchen. Kitchen doors and windows are open and flies are getting into food. Resident had a maggot on the fruit cup. Facility is serving out of date, moldy cheese and crackers. Kitchen is dirty and nasty. Sanitarian performed an inspection/investigation on 7/5/23 (see inspection report dated 7/5/23). I spoke with the new manger regarding the complaint. According to the manager, she just had several employees leave. Sanitarian did not observed doors/windows open at time of inspection. Nor did I observe flies/maggots, and/or out of date food products at time of inspection. However, sanitarian did note several violation on the inspection report (see report). Complaint completed/dismissed.

II - P - Good Hygienic Practices: Food employee was demonstrating good hygiene practices.
III - P - Preventing Contamination by Hands: Hand washing facilities are properly supplied.

IX - P - Highly Susceptible Populations: Unopened food packages are not re-served.
IX - P - Highly Susceptible Population:The operation was serving pasteurized juices to highly susceptible population.
VI - P - TCS Food: Observed hot foods being held at 135°F or above; cold foods being held at 41°F or below.
VI - P - TCS Food: Observed ready to eat TCS foods being properly date marked, and discarded when required.
VII - P - Protection from Contamination: Foods are protected from physical and environmental contamination during storage, preparation, holding and display.
X - P - Chemical: Toxic materials are properly identified and stored.
IV - Demonstration of Knowledge: There was no person in charge present in the food facility during inspection.
