Critical CorrectedDuringInsp Observed the air gap between a plumbing fixture and a piece of equipment that was not sufficient to prevent back siphonage of contaminated water. OBSERVED :drain lines from hot water heater in the floor drain. Provide an air gap to keep contaminated water from contaminating potable water which could cause a food borne illness. CORRECTED: Employee removed lines from floor drain.
To prevent contamination, an air gap between the water supply inlet and the flood level rim of the plumbing fixture, equipment, or nonfood equipment shall be at least twice the diameter of the water supply inlet and may not be less than one inch (25 millimeters). Correct By: 25-Jun-2018

Non-food contact surfaces of equipment are dirty. OBSERVED : DUST/ DIRT on walk in cooler fan guards. To prevent a contamination of food which could cause a foodborne illness, Clean fan guards.
Nonfood-contact surfaces of equipment shall be kept clean.
