[?] A summary of the violations found during the inspection are listed below.

3717-1-03.4(F)(1) / Time/temperature controlled for safety food - hot and cold holding.
Critical TCS foods were not being held at the proper temperature. The chicken in the prep cooler was 46 degree and the meatballs in the walk in cooler was 46.
To prevent the growth of pathogens that could cause a foodborne illness, except during preparation, cooking, or cooling, or when time is used as a public health control, TCS food shall be held at 135°F or above (except that roasts cooked using time and temperature parameters in 3717-1-03.3 may be held at 130° F), or at 41°F or less. ADJUST THE PREP COOLER AND WALK IN COOLER TO 41 DEGREES OR LOWER. UNTIL THE TEMPERATURE RETURNS TO 41 DEGREES OR LESS DO NOT USE THE COOLER FOR TCS FOOD. THE PREP COOLER TCS FOOD WAS MOVED TO THE UNDER COUNTER COOLER AT THE TIME OF INSPECTION. THE MANAGER WAS LOOKING INTO TURNING BACK ON THE AIR CONDITIONER WHICH TIGHT CORRECT THE TEMPERATURE IN THE WALK IN COOLER. Correct By: 05-Nov-2015

Comments
The health department received a call from a customer claiming "they became ill from eating at Subway." They ate on Tuesday (11-3-15) at 6:30 PM and received the food at 6:00PM.
The complaint was discussed with, Lexi Roberts, the manager.
The purpose of the investigation is to determine events or activities that possibly could have contributed to this person becoming ill and so that measures can be taken to control hazards and prevent illnesses.

A discussion with the PIC revealed:
PIC was not aware of the complaint
PIC was not aware of any other ill customers
PIC reported no unusual deliveries
PIC reported no malfunctioning equipment except the air conditioner
PIC reported no employee illness in the last 7 days
PIC reported no problems with the water, electric, sewage back up. or fire
PIC reported not knowing of any employee with cuts or infections on their hands or arms

A temperature log is kept - recorded temperatures for Tuesday 11-3-15 were acceptable.

Observations during investigation:

gloves were worn when marking sandwiches
3 comp sink set up properly
uniform appeared clean
