States he ate there on March 31, 2017 and the coleslaw he ordered was warm and a bug came crawling out by him that had a long slender body that the bartender took care of. KB
Facility shall come up with a cleaning schedule. Dirt & food debris throughout the facility . Looks like facility just shuts down & no one cleans before opening.
Facility had a licensed pest operator (Carothers) Last treatment out of three was on 4/3/17
Coleslaw was made with room temperature mayo that they purchased from Cornells
