Repeat Outdoor refuse storage surface is not properly constructed, and sloped to drain. The dumpster sets on gravel.
To allow for easier cleaning, an outdoor storage surface for refuse, recyclables, or returnables shall be constructed of nonabsorbent material and shall be smooth, durable, and sloped to drain. MOVE THE DUMPSTER TO THE ASPHALT, OR INSTALL A DUMPSTER PAD MADE OF CONCRETE OR ASPHALT THAT EXTENDS AT LEAST 3 FEET FROM THE EDGES OF THE DUMPSTER. Correct By: 26-Sep-2016

3717-1-05.4(F)(1)/Outside receptacles - tight fitting lids
Observed outdoor refuse containers without tight fitting lids, doors, or covers. The dumpster lids were left open.
To preventer attacking pests, receptacles and waste handling units for refuse, recyclables, or returnables used outside with materials containing food residue, shall be designed and constructed to have tight-fitting lids, doors, or covers. KEEP THE DUMPSTER LIDS CLOSED. Correct By: 26-Mar-2016
3717-1-05.1(J)/Backflow prevention device - when required.
Observed no air gap or approved backflow prevention device on the plumbing system. The backflow preventer was taken off the hose bib in the hallway.
To prevent contamination, a plumbing system shall be installed to preclude backflow of a solid, liquid, or gas contaminant into the water supply system at each point of use at the FSO or RFE, including on a hose bibb if a hose is attached or on a hose bibb if a hose is not attached and backflow prevention is required by the Ohio building code, by: (1) Providing an air gap as specified under paragraph (D) of this rule; or (2) Installing an approved backflow prevention device as specified under paragraph (E) of this rule.
