Critical Repeat A thermometer capable of accurately measuring the temperature of thin foods was not available.
A food temperature measuring device with a suitable small-diameter probe shall be provided and readily accessible to measure the temperature of thin foods. REPLACE/REPAIR THE THIN PROBE THERMOMETER. THE EXISTING ONE MAY JUST NEED A NEW BATTERY. THIS IS A CRITICAL BECAUSE ACCURATE FOOD COOKING TEMPERATURES CAN NOT BE CHECKED. Correct By: 21 February-2014

Repeat Nonfood-contact surfaces are not designed and constructed to allow easy cleaning or maintenance. The floor around equipment, especially around the stove was dirty. The stove, freezer and small table by the stove was dirty.
Nonfood-contact surfaces shall be designed and constructed to allow easy cleaning and to facilitate maintenance. CLEAN THE FLOOR AROUND THE FREEZERS, REFRIGERATOR, SMALL TABLE BY THE STOVE, AND THE STOVE. Correct By: February 21, 2014

Observed foods that are from an unapproved source. The fish in the freezer does not have proper identification and appears to be non-commercially obtained.
Foods shall be obtained from sources that comply with law. REMOVE THE FISH IN THE FREEZER IN ZIPLOCK PLASTIC BAGS FROM THE FACILITY. USE FOOD ONLY FROM AN APPROVED SOURCE AND KEEP THE ORGINAL LABELING. THIS IS A CRITICAL VIOLATION BECAUSE FOOD FROM AN UNAPPROVAL SOURCE MAY BE CONTAMINATED. Correct By: 23-Nov-2013

TO AVOID ENFORCEMENT ACTION CLEAN AND PAINT THE EXISTING SHELVES IN THE WALK IN COOLER AND REMOVE THE LINOLEUM, OR REPLACE THE SHELVES BEFORE THE CORRECT BY DATE. THE BOTTOM SHELF MUST BE AT LEAT 6 INCHES OFF OF THE FLOOR. Correct By: 23-Jan-2014
Comments
Several items from the 11-23-13 inspection have been corrected.
-The bags of fish not from a commercial supplier have been discarded.
-The shelves in the walk in cooler have been replaced.
-Thermometers have been placed in both bar coolers.
-The potato slicer is clean and reportedly cleaned daily.
-The stained ceiling tile has been replaced, however, with non-washable type panels.

Because progress has been made the deadline for correction on the remaining items has been extended.
The ceiling tile must be replaced with washable type ceiling tile, but this can be postponed until after the residential equipment is replaced.
