Repeat Observed improper storage of food items. Food boxes were on the floor in the walk in cooler.
Food shall be protected from contamination by storing the food in a clean, dry location, where it is not exposed to splash, dust, or other contamination, and at least six inches above the floor. STORE FOOD, EVEN IF IN BOXES, ON THE SHELVES. Correct By: 03-Mar-2015

Repeat Floors, walls, and/or ceilings were not smooth and easily cleanable. The walls and ceiling in the walk in cooler are rough concrete.
Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. PAINT OR COVER THE WALLS AND CEILING IN THE WALK IN COOLER WITH A SMOOTH DURABLE EASILY CLEANABLE SURFACE. Correct By: 12-May-2015
