Critical Observed working containers of poisonous or toxic chemicals that were not properly labeled with the name of the material. Two spray bottles are not labeled.
To prevent health hazards, working containers used for poisonous or toxic materials such as cleaners and sanitizers taken from bulk supplies shall be clearly and individually identified with the common name of the material. LABEL BOTH SPRAY BOTTLES WITH THE NAME OF THE CONTENTS. Correct By: 20-Feb-2016

3717-1-04.1(KK) / Food equipment - certification and classification.
Repeat Equipment is not approved by a recognized testing agency. The white roaster is not for commercial use.
Food equipment that is acceptable for use in a FSO or RFE shall be approved by a recognized food equipment testing agency. REMOVE THE WHITE ROASTER FROM THE KITCHEN. Correct By: 20-Feb-2016
3717-1-04.1(Y) / Temperature measuring devices.
Cold or hot holding equipment did not contain a thermometer, or the thermometer was not correctly placed. The under counter cooler in the large kitchen room does not have a thermometer.
A properly designed temperature measuring device as specified in this rule shall be located in the warmest part of a cooling unit or the coolest part of a food warming unit. INSTALL A THERMOMETER IN THE UNDERCOUNTER COOLER IN THE LARGE KITCHEN ROOM. Correct By: 27-Feb-2016
3717-1-04.3(B) / Fixed equipment installation - spacing or sealing.
Repeat Fixed equipment did not appear to be properly sealed or spaced for cleaning.
Equipment that is fixed shall be installed so that it is spaced to allow access for cleaning, from adjoining equipment, walls, and ceilings a distance of not more than one thirty-second inch or one millimeter; or sealed to adjoining equipment or walls, if the equipment is exposed to spillage or seepage; Counter-mounted equipment shall be installed to allow cleaning of the equipment and areas underneath and around the equipment as specified in this rule. -the gasket on the 2 door freezer is intact.
