Repeat The physical facilities are not being maintained in good repair. Door seal at the back door is loose which is creating an entry for pests.
The physical facilities shall be maintained in good repair. REPAIR THE DOOR SEAL AT THE BOTTOM OF THE BACK DOOR TO PREVENT PESTS FROM ENTERING THE BUILDING. Correct By: 08-Sep-2016

Repeat Observed mops being dried using an unapproved method. Mop was soiling the wall.
After use, mops shall be placed in a position that allows them to air-dry without soiling walls, equipment, or supplies. INSTALL A MOP HANGER SO THE MOPS CAN DRY WITHOUT SOILING THE WALL. Correct By: 08-Sep-2016

TCS foods were not being held at the proper temperature. The pies at the front of the store were 46 degrees.
To prevent the growth of pathogens, except during preparation, cooking, or cooling, or when time is used as a public health control, TCS food shall be held at 135°F or above (except that roasts cooked using time and temperature parameters in 3717-1-03.3 may be held at 130° F), or at 41°F or less. ADJUST THE COOLER TO 41 DEGREES. MOVE THE CREAM PIES AND OTHER TCS FOOD IF TEMPERATURE IS NOT 41 DEGREES OR LESS. Correct By: 01-Sep-2016
