Critical An RFE was constructed, altered, and/or equipment was added without prior approval from the licensor.
To prevent foodborne illness, no person, firm, association, organization, corporation, or government operation shall construct, install, provide, equip, or extensively alter a retail food establishment until the facility layout and equipment specifications therefore have been submitted to and approved in writing by the licensor. When the facility layout and equipment specifications are submitted to the licensor, they shall be acted upon within thirty calendar days after date of receipt. The facility layout and equipment specifications shall be approved by the licensor in accordance with the rules adopted pursuant to section 3717.05 of the Revised Code stating the criteria for facility layout and equipment specifications. A new walk in cooler for beer was added to the facility. The cooler appears accessible. Reportedly, the plumbing and building departments have given their approvals of the new cooler. Reportedly, more additions and remodeling is planned. The facilities person was advised to submit plans to the Shelby City Health Department for review. Additional food items will be sold including hot dogs. A 3 compartment sink, hand sink, durable smooth walls, floors and ceiling, and approved equipment must be used. Reportedly, no fruit or vegetables will be prepared. Approvals from the building, fire, and plumbing departments and approval from the Shelby City Health Department must be obtained before use of any remodeled/additions. Please submit plans, a menu (or new food items to be sold), a list of the new equipment with make and model numbers, and a complete plan review application. There is no fee for the plan review from the Shelby City Health Department. Obtain permits from the plumbing and building departments if they haven't been obtained yet. Submit a copy of the written approvals from the building, fire and plumbing departments.
