The physical facilities are not being maintained in good repair. OBSERVED: Water damaged dropped ceiling tiles throughout facility and missing floor tiles . Repair roof from leaking and replace water damaged ceiling tiles to prevent a contamination from the premises which could cause a foodborne illness and replace missing floor tiles
