Critical Water clarity was not sufficient to see the pool bottom (or Secchi disc), please ensure pool is clear enough to see the main drain prior to allowing bathers into the pool.
Do not operate the pool when water clarity is poor.

Ladders/stairs/steps were not in good repair or were not slip resistant. Ladder is loose.
Repair or replace the ladders/stairs/steps so that they are slip resistant and in good repair.
3701-31-04 (B)(6)(p) / Deck surfaces are free of standing water or other hazards.
Deck surfaces had standing water or other hazards. Tie downs for pool cover need to be lowered to not be a hazard.
Repair areas of the deck where standing water is occuring. If more than 50% of the deck must be replaced, plans must be submitted to the Ohio Department of Health for review and approval.
