Repeat pH was outside the range of 7.2 to 7.8. Observed at 8.0

Repeat Emergency device (telephone) was not working. No dial tone.

Have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve. Correct By: 26-Jul-2023

Follow up inspection. ACC fixed, flow meter stuck but there is circulation of system. Slightly high pH, slightly low ORP, emergency telephone not working. Other parameters were within the required ranges at time of follow-up inspection. No other problems noted. AT
