A USCG Type IV personal flotation device with 30 -60 foot throw line was not provided at the unguarded pool. Throw line for ring buoy not long enough.
Provide a proper floatation device at all times.
3701-31-04 (E)(2)(b)(i) / 12 ft. non-telescopic reach pole with Sheppard's crook & a USCG Type IV personal flotation device with 30 - 60 foot throw line.
12 ft. non-telescopic reach pole with Shepherd's crook was not provided at the pool. Shepherd's crook attached to a pole which is too short, the crook is also not secured to the pole.
Provide the non-telescopic reach pole and personal floatation device at the pool at all times.
