[?] A summary of the violations found during the inspection are listed below.

3701-31-04 (B)(6)(h) / Required meters and gauges maintained/functional
Corrected During Inspection Required meters and gauges were not maintained or were not functional while the pool is in operation. Observed flow meter was stuck at the time of inspection. PIC called operations manager to backwash the filter. Once backwash was completed, the flow meter worked again and recorded 973 GPM.
Maintain all meters and gauges in a properly funtioning manner at all times while the pool is in operation.
3701-31-04 (B)(6)(p) / Deck surfaces are free of standing water or other hazards.
Deck surfaces had standing water or other hazards. In the corner of the pool deck, the pavers had been raised to create a tripping hazard. There was also an electrical box that was not flush with the pavers that created a tripping hazard.
Repair areas of the deck where standing water is occuring. If more than 50% of the deck must be replaced, plans must be submitted to the Ohio Department of Health for review and approval.
3701-31-04 (C)(3) / Total alkalinity is at or greater than 60 ppm
Repeat Total alkalinity was less than 60 ppm.
Raise the total alkalinity to at least 60 ppm.
3701-31-04 (D)(7)(b) / pH and ORP values shall be displayed.
Critical ORP value was not displayed on the ACC unit. The settings of the unit had been changed to no longer display ORP, and nobody on site was able to access the correct setting. Change the settings back so that ORP is displayed.
Have the ACC unit serviced or replaced so that the ORP value is displayed
3701-31-04 (D)(8)(c) / Test kit reagents fresh/ properly stored.
Test kit reagents were not fresh or stored properly. R-007 was expired. PIC replaced R-007 at the time of inspection. Facility also refills 870, 871, and R-004. The large bottles used for Refills were not expired at the time of inspection.
Obtain fresh reagents and store them properly.
3701-31-04 (E)(2)(a)(i) / 12 ft. non-telescopic reach pole with shepherd's crook is provided
12 ft. non-telescopic reach pole with shepherd's crook was not provided at the unguarded pool. Observed the shepherd's crook, but the hook was unsecured and would not properly attached to the pole. Correction: repair or replace the shepherd's crook.
Provide a 12 ft. non-telescopic reach pole in the pool area at all times.
3701-31-04 (E)(2)(a)(ii) / A USCG Type IV personal flotation device with 30 -60 foot throw line provided
A USCG Type IV personal flotation device with 30 -60 foot throw line was not provided at the unguarded pool. Observed the personal flotation device, did not observe the 30-60 foot throw line. Correction: obtain a 30-60 foot throw line.
Provide a proper floatation device at all times.

Facility had purchased new drain covers and were going to go about installing these new covers, and were informed to keep records of when drains were installed, and the expiration date as per the manufacturer. The emergency phone for the outside pool, and the indoor pool are not working. The emergency phone for the outside FSO was working at the time of inspection.
