Public swimming pool was constructed, installed, or substantially altered without obtaining approval from the Ohio Department of Health (ODH). New disinfection system installed, please complete Equipment Replacement Notification form and submit to ODH.
Contact ODH to obtain approval for any construction/alterations before operating pool.
