3701-31-04 (B)(4) / The licensee /authorized representative is keeping records
The licensee /authorized representative was not keeping or maintaining operational records. Disinfection residuals are taken twice a day; however, records are only recorded once a day.
All facility records must be maintained and available for review upon request.
