3701-31-04 ( D )(6)(a) / ORP reading shall be 650 millivolts or greater.
Repeat ORP reading was below 650 mv. The auto controller alarm was activated and the reading was 0 mv. The PIC stated, a work order for repair has been placed and scheduled. Please monitor closely until repaired.
Have the ACC unit inspected to determined why the ORP is low. Correct By: 29-Apr-2022

3701-31-04 (B)(4)(a)(vi)/Total alkalinity is tested weekly
Total alkalinity was not tested weekly or recorded. Note: I went over this with Parth and he will educate the staff to make certain it is recorded, even if he is not there.
Test total alkalinity at least once/week. Correct By: 29-Apr-2022
3701-31-04(D)(6)(a)(4)/Spa free chlorine disinfectant residual shall be above 2 ppm.
Spa free chlorine disinfectant residual was below 2 ppm. The fee chlorine was 0 ppm. The PIC immediately closed spa. He is going to drain spa, clean tile, refill, then verify reading meet requirement before opening. Please contact me at kcooper@wcchd.com or 513-695-1472 once corrected.
Do not operate the spa until the chlorine disinfectant residual is at 2 ppm or above. Correct By: 01-Apr-2022
