3701-31-04 (B)(6)(p) / Deck surfaces are free of standing water or other hazards.
Deck surfaces have some hazards. The concrete is broken and missing in areas near the pool edge and near hand rail.
Repair areas of the deck where standing water is occurring or in need of repair. If more than 50% of the deck must be replaced, plans must be submitted to the Ohio Department of Health for review and approval.
3701-31-04 (B)(6)(q) / Depth markers/deck warning signs are maintained
Depth markers/deck warning signs were not maintained in a clearly legible/visible manner. The "no diving" and "depth markers" are faded.
Replace or repaint the depth markers/deck warning signs so that bathers are able to clearly see the depth of the water and that no diving is permitted where clearly indicated.
3701-31-04 (B)(6)(s) / Perimeter barrier is in good repair to prevent unauthorized entry
The main gate did not self-close.

3701-31-04 (E)(2)(a)(ii) / A USCG Type IV personal flotation device with 30 -60 foot throw line provided
A USCG Type IV personal flotation device with 30 -60 foot throw line was not being maintained at the unguarded pool. The rope on the ring buoy was frayed and has broke. The PIC will order a new ring buoy with new rope.
Provide a proper floatation device at all times.
3701-31-04 (E)(2)(e) / Working emergency telephone/call box provided.
Emergency device (telephone) was not working.
Ensure that the emergency device is functioning properly and rings to the emergency response authorities.
