Repeat Equipment components are not intact, tight or properly adjusted. The cold holding units have damaged lids, gaskets, and shelving.

Repeat Equipment not approved by a recognized testing agency. The Coca Cola cold holding unit is not approved to store TCS foods.

Repeat Physical facilities not maintained in good repair. Multiple areas throughout the kitchen no longer have walls that are smooth, durable, and easily cleanable (main preparation area, bar, mop area, storage area, and near dough mixer).

Repeat Facility not maintained clean. Floors are not clean in the kitchen and bar under equipment.

3717-1-02.4(C)(9)/PIC: duties - ensure employees are properly maintaining temperatures of TCS foods during hot and cold storage
PIC did not ensure employees are properly maintaining temperatures of TCS foods during hot and cold storage. Nacho cheese was not hot holding at proper temperatures and the spicy garlic was not properly time stamped when using the TILT (time in lieu of temperature) policy. Education was provided to improve food safety.
3717-1-02.4(B)(2)(m)/PIC: Demonstration of Knowledge - Identifcation and proper use of poisonous or toxic materials
PIC unable to demonstrate knowledge of storage and use of toxic materials. A spray can of pesticide was improperly stored at the bar and being used improperly to reduce the insect population at the bar. Education was provided to the PIC about proper pest control procedures.
3717-1-02.4(B)(2)(h)/PIC: Demonstration of Knowledge - Describing relationship between prevention of foodborne illness and management of contamination, hand contact with food, hand washing, and clean facility
PIC unable to demonstrate knowledge of preventing foodborne illness by controlling contamination, personal hygiene, and facility maintenance. Staff is not properly washing hands and the lack of proper cleaning may lead to contamination. Education was provided to the PIC about proper hand washing, cleaning, and personal hygiene.

Food employee not washing hands in approved handwashing sink. The employee washed hands in the food preparation sink. Education was provided to improve food safety.

Handwashing signs not posted in the restrooms.
3717-1-04.5(A)(1)/Cleanliness of equipment food-contact surfaces and utensils.
Equipment food-contact surfaces or utensils are unclean. Multiple pieces of equipment are not clean (dough mixer, can opener blade, sauce bottles, knife block). The PIC began to clean the equipment and education was provided to the staff about proper cleaning procedures.
3717-1-03.4(F)(1)(a)/Time/temperature controlled for safety food - hot holding.
TCS foods not being hot held at the proper temperature. Nacho cheese was holding at 114 degrees F at time of inspection. The PIC voluntarily discarded the cheese once the violation was identified. The PIC stated that nacho cheese will not be used until the hot holding equipment can be repaired/replaced and properly hold cheese over 135 degrees F.
3717-1-03.4(I)(2)/Time as a public health control - four hour time limit
Improper use of time as a public health control (4 hours). A TILT (time in lieu of temperature) policy was being used improperly. A bottle of spicy garlic sauce was not properly time stamped for four hours and was holding at 67 degrees F near the grill. The PIC voluntarily discarded the product once the violation was identified. Education was provided to the staff to improve food safety regarding the use of the TILT policy.

Improper use of a poisonous or toxic substance. A can of pesticide spray was being used at the bar in an attempt to control the drain fly population. The PIC discarded the can and education was provided to the staff about proper pest control procedures.
3717-1-07.1(A)/Poisonous or toxic materials: Storage: separation.
Improper storage of poisonous or toxic materials. A spray can of pesticide was stored improperly at the bar near the 3-compartment sink. The PIC immediately remove the can once the violation was identified. Education was provided to improve food safety.
3717-1-06.4(K)/Controlling pests.
Presence of live insects (drain flies) in the kitchen and at the bar. The PIC has hired a licensed pest control contractor to treat. Education was provided to reduce insect population (cleaning, etc.).
