3701-31-04 (A) / All construction or alterations of a public swimming pool are approved
Repeat New disinfection system installed and has not been approved.
Submit Equipment Replacement Notification Form to Ohio Department of Health for approval.
3701-31-04 (B)(4) / The licensee /authorized representative is keeping records
Repeat The licensee /authorized representative was not keeping or maintaining operational records. No results recorded for total alkalinity test. Total alkalinity must be tested, and the results recorded, at least once per week. No records for test on June 27 afternoon or June 28 morning. The pool must be tested each morning prior to bathers entering the pool and every 4 hours that the pool is open.
All facility records must be maintained and available for review upon request.
3701-31-04 (C)(2) / pH is within 7.2 to 7.8
pH was outside the range of 7.2 to 7.8.
Treat the pool water so that the pH is within the acceptable range.
3701-31-04 (D)(05) / Cyanuric acid level shall not exceed 70 ppm.
Repeat Cyanuric acid level exceeded 70 ppm.
Drain the pool at least halfway and fill with clean water. Monitor the amount of cyanuric acid being used and adjust accordingly.
3701-31-04(D)(6)(a)(1) / Combined chlorine shall not exceed 1.0 ppm.
Combined chlorine was greater than 1.0 ppm.
Treat the pool so that the combined chlorine is less than 1 ppm.

Chlorine disinfectant residual was less than 1 ppm for the pool.
Do not operate the pool when the disinfectant residuals are less than what is required. Ensure the chlorine disinfectant residual is at 1 ppm or above prior to placing the pool back into operation.
