Required meters and gauges were not maintained or were not functional while the pool is in operation. Flow meter broke. New one has arrived. Please install so that flow rate can be measured
Maintain all meters and gauges in a properly funtioning manner at all times while the pool is in operation.

Repeat Secondary means of entrapment/evisceration prevention was not provided. Must install secondary means to prevent entrapment.
Submit plans to the Ohio Department of Health for review and approval for the secondary means that will be employed on the pool.
