Have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve.

Depth markers/deck warning signs were not maintained in a clearly legible/visible manner.
Replace the depth markers/deck warning signs so that bathers are able to clearly see the depth of the water and that no diving is permitted where clearly indicated.
3701-31-04 (E)(2)(a)(ii) / A USCG Type IV personal flotation device with 30 -60 foot throw line provided
30 -60 foot throw line was unattached and in poor repair.
Provide a throw line at all times pool is operational.

Emergency device (telephone) was not working.
Ensure that the emergency device is functioning properly and rings to the emergency response authorities.

Flow was below minimum requirement, please have circulation system inspected. Emergency phone was not plugged in at time of inspection, please plug it in. Depth markers are faded, please re-paint them. Throw line was unattached from buoy and in poor repair, please replace and re-tie the throw line. Alkalinity was not tested weekly, please ensure that it is being tested and recorded. KJS
